1. The Course Provider List is updated periodically. For updated Course Provider information go to the Course Provider page.
2. The HCD OL 8016 Live Scan Application form needs to be in triplicate (a copy for the live scan operator, a copy for HCD, and a copy for your records). You can either download the form three (3) times or make additional copies of the form after it has been downloaded.
